adding a form with date selection to content pages

Message Topic Search Topic Options Post Reply Create New Topic Printable Version Translate Topic

   I am trying to add a form to one of my content pages, and I don;t see how to add it.
I want it to have 
Name: "text field"
Date you would like to have your party: "pop up date selection tool"
How did you hear about us? "drop down box "

SUBMIT

and have this go to my email address, with a subject line of "new trunk show inquiry"

Does anyone know how to do this?
Or can someone recommend a developer who can do this?
