Does anyone know of a good freelancer, or company with good industry rates that will install, configur and customize product cart (specifically a BTO product cart in this case)?

This is just a suggestion. When I am looking for help with software, I first look for the independent programmers who contribute actively to the forums for the product. NetProfits http://www.nicwebdesign.com is only second to Early Impact for the number of posts on this forum. It is always very helpful and they frequently post code to assist store owners (ie: free help that could/should be charged for). They obviously have a serious interest in the guts of the software and know it very well. Number two place to search is here: http://www.earlyimpact.com/partners.asp Just my 2 cents.
