adding text box - fields to item

Referring to this item... now that the color has been solved... now we need to have to text fields - to input golf courses.. so where in wiki do i read to add this... its like this..

TEXT BOX

Golf Course 2
TEXT

then they type in the box and this is send with thier order... or perhpas this is another attribute related to the color?
