Guys,
Is there "any" way in product cart (v4) to accomplish this scenario?
1. We would like to charge a "setup fee" addition to the online price if the product is "out of stock". We would ideally want to be able to change the fee by product category. Is this possible?

Not with stock code, but it could certainly occur as a result of custom code.
We recently completed a mod for a client selling sausage. When the system detects that products in the cart are perishable, various business rules are enforced, among them the addition of a handling fee.

Similarly, your "out of stock" items could be the trigger for placing a fee onto the order during the checkout process. Obviously your category admin would need to be altered to support the variable that you wish to charge in such instances.
