adding fields to checkout

   Just launched vegaspartypass.com...  need some help from people that have customized before. 

I have customized the heck out of this thing, and now I need to add 4 fields on the order level, not the product level, up checkout.  Has anyone done this?  I figure I can take the code format for the delivery date/referral box and make that work for that I need, but does anyone have a better way?
