Incomplete orders issue

Hello,

We just updated our store to version 4.1SP2 and have noticed an increase in incomplete orders. We know that is to be expected with PC version 4.x compared to 3.x, but we have encountered a situation we've never seen before.

Today an order was placed in the store that was marked as incomplete, but we received the credit card authorization confirmation from Authorize.net. We manually updated the order status to pending, but we were wondering what might have caused that to happen? We are also contacting the customer to make sure they know the order was successfully completed.

Has anyone seen this before?
