Is there a tutorial to creating a customized form?

   I want to create a new form based off of the contact us form. 
Basically, I want to add a few more fields:

First Name 
Last Name 
Company Name 
Website
Email 
Phone 
Comments 

I opened the contact.asp form in dreamweaver but the code just confuses me.  It would be great if someone could provide step by step instructions on how to do this.  Thanks in advance
